Carers play an important role in supporting people through their healthcare journey. They often help with appointments, decision-making, understanding information, managing treatment and providing ongoing support at home.
At the South Metropolitan Health Service (SMHS), we are committed to recognising, involving and supporting carers. Understanding carers' experiences helps us identify what we are doing well and where we can improve.
This survey asks about your experiences supporting someone who has used hospitals and health services within the SMHS, including Fiona Stanley Hospital, Fremantle Hospital, Rockingham General Hospital, Peel Health Campus, Cockburn Health, Murray District Hospital and our Community Health Services.
Your feedback will help us:
- better understand the needs and experiences of carers
- identify opportunities to improve how we communicate and work with carers
- strengthen how carers are recognised, involved and supported across our health services.
Thank you for taking the time to share your experience.
