About the Disability Health Profile Form

The Disability Health Profile form is a paper-based form that is added to a person's medical record.

It has been created to allow patients or carers of a loved one with a disability, let staff know information about their disability and the ways that staff can support them.

This information helps staff to provide person-centred care and for patients and family to have a great patient experience.

If you or the person you care for has a disability, please request a form from a staff member or download the form by clicking on the link below.

SMHS is a Hidden Disability Sunflower Organisation.

Not all disability is visible.

Some patients may come to the hospital with a Sunflower lanyard or badge to show that they have a hidden disability.

This shows the staff at the hospital that they may need more help or more time.

Staff have done training on the Hidden Disability Sunflower and wear the "Proud to Support the Sunflower" badge or lanyard.

The training helps staff know what to say and how to support a person with a hidden disability.

Hospital Stay Guidelines

The Hospital Stay Guidelines aim to inform people with disability and their support networks, disability service providers and hospital staff of their respective roles and responsibilties throughout a person's stay in hospital in WA.
